Birthday Announcement on the Marquee

Celebrate your student's special day on the school marquee!  Print out and fill out the BIRTHDAY REQUEST form and return it along with $5 cash or check (made out to Mather Heights PTA) to the drop box in front of the school. 

**PLEASE MAKE SURE YOUR REQUEST IS SUBMITTED THE WEEK BEFORE YOUR STUDENTS BIRTHDAY AT THE LATEST**
